SUMMARY:Ethical Considerations for Using AI to Predict Suicide Risk: ASBH Conference
DESCRIPTION:This flash presentation by Hastings Center PMRA Faith Wershba at the annual ASBH Conference will consider using AI to predict a patient’s suicide risk. \n\n\n\nAbstract for the presentation: Predicting a patient’s suicide risk is a complex and high-stakes task. While there are a number of known risk factors that correlate with suicide attempts at the population level\, the presence or absence of a given risk factor may not reliably predict an individual’s risk of attempting suicide. In an attempt to improve suicide risk prediction at the individual level\, a number of researchers have developed machine learning algorithms which can be applied to patient medical records to evaluate suicide risk. Currently\, such algorithms are not used as part of clinical practice. However\, if they prove successful\, many experts think that these AI systems could—and should—be integrated into clinical care to aid suicide prevention efforts. Indeed\, if these algorithms are found to increase predictive accuracy compared to clinician judgement\, it seems ethically desirable to promote their use in clinical settings. However\, this possibility raises ethical considerations regarding disclosure\, consent\, and data use. If it becomes standard practice to screen patients’ electronic health records using an automated suicide risk prediction algorithm\, does a healthcare institution have a duty to disclose this fact to their patients? Moreover\, should applying these algorithms to patient medical records require patient consent? If so\, should such consent be opt-in or opt-out? Lastly\, if suicide risk predictions generated in the clinical setting are used to further train or refine the algorithm\, should patients be consulted about the use of their personal data in this way?

DESCRIPTION:Panel Presentation: A Dialogue on the Works of Nancy Dubler\, with Hastings Center Research Associate Julia Kolak. \n\n\n\nPanel:Autumn Fiester\, PhDJoseph Fins\, MDJulia Kolak\, PhD\, HEC-CJu Zhang\, PhD \n\n\n\nAbstract: Bioethicist scholar Nancy Neveloff Dubler was a force of nature. As a lawyer turned bioethicist\, she was a highly impactful scholar of clinical ethics\, who pursued justice as an advocate for the marginalized and vulnerable. In 2004\, she co-authored the landmark work Bioethics Mediation\, pioneering a novel approach for bedside conflicts. In this panel\, three presenters explore her scholarly legacy to the field of bioethics. The first presenter explores Dubler’s transformative contributions to ethics consultation. The presenter argues that Dubler’s Distinctive methodology resists the epistemic harms of “speaking for” patients by fostering an inclusive process of dialogic engagement. Contrasting this approach with traditional consultation models\, this talk examines how the tools of bioethics mediation create space for “speaking with” stakeholders\, embodying a deliberative\, participatory\, and context-sensitive process of HCEC. The second presenter explores Dubler’s opposition to “benevolent deception\,” arguing that it compromises the integrity of ethics consultants\, erodes trust\, and distorts ethical mediation. This talk reaffirms her vision by proposing a trust-oriented approach that fosters shared deliberation and cultural respect when caring for patients from cultures that endorse benevolent deception. In the third presentation\, the speaker will argue that the best way to protect Dubler’s legacy of “bioethics mediation” is to relocate the function of mediation from ethics consultation services to the offices of patient & guest relations. Given the recent shift in ethics consultation practice away from interpersonal facilitation to physician-focused recommendation\, the widespread need for clinical conflict management is unlikely to be met by a hospital’s ethics service.

Dr. Fins is the E. William Davis\, Jr. M.D. Professor of Medical Ethics and Chief of the Division of Medical Ethics at Weill Cornell Medicine where he is a Tenured Professor of Medicine\, Professor of Medicine in Psychiatry\, Professor of Medical Ethics in Neurology\, Professor of Medical Ethics in Rehabilitation Medicine\, and Professor of Health Care Policy and Research. He is the founding Chair of the Ethics Committee of New York-Presbyterian Weill Cornell Medical Center where he is an Attending Physician and Director of Medical Ethics. A member of the Adjunct Faculty of Rockefeller University and Senior Attending Physician at The Rockefeller University Hospital\, he co-directs the Consortium for the Advanced Study of Brain Injury (CASBI) at Weill Cornell Medicine and Rockefeller. He is currently a Visiting Professor of Law at Yale Law School in addition to his appointment at Weill Cornell Medicine. \n\n\n\nDr. Fins is an elected Member of the National Academy of Medicine (formerly the Institute of Medicine) of the National Academies of Sciences\, a Fellow of the American Academy of Arts and Sciences\, and by Royal Appointment an Academico de Honor (Honored Academic) of the Real Academia Nacional de Medicina de España (the Royal National Academy of Medicine of Spain). In 2022\, he was elected to the Association of American Physicians. \n\n\n\nDr. Fins graduated from Wesleyan University (B.A. with Honors\, The College of Letters\, 1982) and Cornell University Medical College (M.D.\, 1986). He completed his residency in Internal Medicine and Fellowship in General Internal Medicine at The New York Hospital-Cornell Medical Center and has served as Associate for Medicine at The Hastings Center. He is a Diplomat of the American Board of Internal Medicine. DESCRIPTION:Hastings Center scholar Carolyn Neuhaus will moderate a panel for the Mullan Institute’s Health Workforce Equity Summit: Rebuilding Health Workers’ Trust in Healthcare Organizations. \n\n\n\nStudies show a steady decline in health workers’ trust in organizations\, and this decline is significantly associated with a reduction in patient trust in clinicians. To date\, most approaches to rebuilding health worker trust have emphasized individual-level interventions and support for leaders. DESCRIPTION:Hastings Center senior research scholar Nancy Berlinger will present “What Do We Have in Common?: Thinking Together About Good Lives for Older Adults and Caregivers in Ageing Societies”  in Seminar II of the Chinese University of Hong Kong Centre for Bioethics’ 10th Anniversary Celebration. \n\n\n\nPopulation ageing—a rise in the average age of a population—results from the combination of public health improvements and biomedical progress that\, over time\, prevent\, treat\, or control infectious diseases and life-threatening health conditions\, plus lower birthrates reflecting changes in the lives and choices of women and families. Ageing societies\, in which more people are over age 65 than under age15\, include wealthy\, technologically advanced nations in East Asia\, Asia-Pacific\, North America\, and Western Europe. A 2023 report from the International Monetary Fund (IMF) describes population ageing as the top demographic trend globally\, as lifespans increase and birthrates decline in all regions of the world. This trend is often framed as a problem: Who will care for all of us as we age? These facts and trends point to an urgent need for researchers and practitioners in ageing societies in different regions to think together about shared challenges. This seminar draw on recent work by The Hastings Center to reflect on how perceptions of difference\, such as stark contrasts between “East” and“West\,” can impede recognition of similar values\, approaches\, and problems as well as meaningful differences. DESCRIPTION:Hastings Center President Vardit Ravitsky will present “Current and Emerging Prenatal Testing Technologies: Bioethical Implications” at the Chinese University of Hong Kong Centre for Bioethics’ 10th Anniversary Celebration. \n\n\n\nPrenatal genetic testing technologies can provide individuals and families with valuable information about their potential prospective children. Such technologies\, including pre-implantation genetic testing of in-vitro embryos and testing of fetuses during pregnancy\, can promote informed decision-making and in some cases can be medically beneficial. They have also been raising powerful bioethical concerns for decades\, such as their impact on parental autonomy\, societal eugenic tendencies\, and disability rights. Recent new technologies\, such as noninvasive prenatal testing (that was developed by Prof. Dennis Lo in Hong Kong) or polygenic screening of in-vitro embryos\, have further stimulated the bioethical discussion regarding what conditions and traits these technologies are targeting. This talk will focus on the recent analysis of prenatal testing in the Western bioethics literature\, as well as cultural and policy implications for East Asia and Hong Kong. The talk will address the notion of a ‘serious’ genetic condition as justifying testing\, and the debate surrounding the use of prenatal testing for medical versus non-medical purposes.

What are the responsibilities of policymakers in the region and abroad? \n\n\n\nThese are among the questions that will be discussed in a webinar on Thursday\, February 13\, at noon EST. \n\n\n\nPanelists: \n\n\n\nGeorgiana Gordon-Strachan\, PhD\, is director of the Tropical Metabolism Research Unit at Caribbean Institute for Health Research at the University of the West Indies in Kingston\, Jamaica. DESCRIPTION:In this webinar at the Steve Biko Centre for Bioethics\, Hastings Center President Vardit Ravitsky and colleagues argue that war is a public health crisis that bioethics has dealt with in too limited a manner. They propose several principles for addressing war as a public health crisis and suggest ways in which these principles can be put into practice. SUMMARY:Changing Life as We Know It–Exploring the Role of Genetic Modification and Our Understanding of Humanity
DESCRIPTION:Hastings Center President Vardit Ravitsky will be a keynote speaker for the FASPE (Fellowships at Auschwitz for the Study of Professional Ethics) Symposium. \n\n\n\nWhat are the ethical issues surrounding genetic modification? How do we define – and regulate – the line between benefit and harm?  In what ways do these possibilities force us to make decisions about the value of life and impose judgments about what makes a life worth living – or even what is considered “normal”?  We will discuss an example in history illustrative of the ways in which assigning genetic value engendered horrific results\, wherein Nazi eugenicists and government policy makers identified those considered a genetic and financial burden on the population – and aimed through sterilization and\, even (euphemistically)\, euthanasia to eliminate  “life unworthy of life”.  In what ways does this history caution us? We will focus on several scenarios wherein genetic modification presents complicated questions around intended and unintended possibilities and consequences – and ultimately how we understand what it means to be human.  \n\n\n\nThe program will serve to examine the ethical issues not just within medicine\, but implicated within theology (and therefore the clergy)\, business (the ethical implications of commercialization)\, law (exploring the legal/regulatory roles\, including recognizing the limitations imposed by the structural irrelevance of national borders)\, journalism (how it is described and written about will be fundamentally important in shaping understanding)\, and advances and uses of technology.
