SUMMARY:Ethical Considerations for Using AI to Predict Suicide Risk: ASBH Conference
DESCRIPTION:This flash presentation by Hastings Center PMRA Faith Wershba at the annual ASBH Conference will consider using AI to predict a patient’s suicide risk. \n\n\n\nAbstract for the presentation: Predicting a patient’s suicide risk is a complex and high-stakes task. While there are a number of known risk factors that correlate with suicide attempts at the population level\, the presence or absence of a given risk factor may not reliably predict an individual’s risk of attempting suicide. In an attempt to improve suicide risk prediction at the individual level\, a number of researchers have developed machine learning algorithms which can be applied to patient medical records to evaluate suicide risk. Currently\, such algorithms are not used as part of clinical practice. However\, if they prove successful\, many experts think that these AI systems could—and should—be integrated into clinical care to aid suicide prevention efforts. Indeed\, if these algorithms are found to increase predictive accuracy compared to clinician judgement\, it seems ethically desirable to promote their use in clinical settings. However\, this possibility raises ethical considerations regarding disclosure\, consent\, and data use. If it becomes standard practice to screen patients’ electronic health records using an automated suicide risk prediction algorithm\, does a healthcare institution have a duty to disclose this fact to their patients? Moreover\, should applying these algorithms to patient medical records require patient consent? If so\, should such consent be opt-in or opt-out? Lastly\, if suicide risk predictions generated in the clinical setting are used to further train or refine the algorithm\, should patients be consulted about the use of their personal data in this way?

DESCRIPTION:Panel Presentation: A Dialogue on the Works of Nancy Dubler\, with Hastings Center Research Associate Julia Kolak. \n\n\n\nPanel:Autumn Fiester\, PhDJoseph Fins\, MDJulia Kolak\, PhD\, HEC-CJu Zhang\, PhD \n\n\n\nAbstract: Bioethicist scholar Nancy Neveloff Dubler was a force of nature. As a lawyer turned bioethicist\, she was a highly impactful scholar of clinical ethics\, who pursued justice as an advocate for the marginalized and vulnerable. In 2004\, she co-authored the landmark work Bioethics Mediation\, pioneering a novel approach for bedside conflicts. In this panel\, three presenters explore her scholarly legacy to the field of bioethics. The first presenter explores Dubler’s transformative contributions to ethics consultation. The presenter argues that Dubler’s Distinctive methodology resists the epistemic harms of “speaking for” patients by fostering an inclusive process of dialogic engagement. Contrasting this approach with traditional consultation models\, this talk examines how the tools of bioethics mediation create space for “speaking with” stakeholders\, embodying a deliberative\, participatory\, and context-sensitive process of HCEC. The second presenter explores Dubler’s opposition to “benevolent deception\,” arguing that it compromises the integrity of ethics consultants\, erodes trust\, and distorts ethical mediation. This talk reaffirms her vision by proposing a trust-oriented approach that fosters shared deliberation and cultural respect when caring for patients from cultures that endorse benevolent deception. In the third presentation\, the speaker will argue that the best way to protect Dubler’s legacy of “bioethics mediation” is to relocate the function of mediation from ethics consultation services to the offices of patient & guest relations. Given the recent shift in ethics consultation practice away from interpersonal facilitation to physician-focused recommendation\, the widespread need for clinical conflict management is unlikely to be met by a hospital’s ethics service.
